This report is for information purposes only and does not replace LEA's normal procedures for reporting defects, which are
to be strictly adhered to.
The report will be automatically emailed to Maintenance, Managemement, Karl, Glyn and Fleet Manager. A copy will also be sent
to the email address entered on the form.
